This prospective study evaluated the efficacy of topical nepafenac combined with oral acetazolamide in the treatment of postoperative macular edema (PME) following uncomplicated phacoemulsification among 1,000 patients. 

PME developed in 92 eyes of which15 were included in the detailed analysis. Significant improvements in best-corrected visual acuity (BCVA) and central macular thickness (CMT) were observed during follow-up visits. First-line treatment with topical nepafenac and oral acetazolamide achieved complete resolution in 73.3% of cases. Four patients who did not respond to initial therapy required subtenon triamcinolone, while only one patient subsequently required intravitreal therapy. Diabetes mellitus and higher cumulative dissipated energy during surgery were significantly associated with poor treatment response, identifying them as key predictors of persistent PME.
