Purpose

To evaluate the effect of simvastatin on CAD progression in patients with normal levels of cholesterol

Design

Randomized, double-blind, placebo-controlled study .

Patients

460 patients with normal levels (4.1-6.2 mmol/L) of cholesterol and established CAD.

Follow-up

Four years

Simvastatin vs. placebo

Results

  • Simvastatin slowed the progression of CAD.
  • The need for revascularisation was reduced by 53.6% in the simvastatin group; need for angioplasty was reduced by 61.9%.
  • Simvastatin caused a greater reduction in lipids as compared to placebo (LDL: 30.9% reduction vs 3.5% increase in placebo group; total cholesterol: 20.7% reduction vs 3.4% increase with placebo; triglycerides: 9.7% reduction vs 8.8% increase with placebo; HDL: 8.2% increase vs 3.1% in placebo).
  • Addition of enalapril to the treatment regimen had a neutral impact on the progression of CAD.

Conclusion

Simvastatin slows the progression of CAD in patients with normal levels of cholesterol and established CAD
