Benefits of Platelet-rich Plasma Therapy in The Treatment of Men with Erectile Dysfunction

Introduction
The prevalence of erectile dysfunction (ED) is increasing and has an adverse effect on the marital life of men. Many therapies such as intracavernosal injection, hormone therapy, phosphodiesterase 5 inhibitors and vacuum erection devices are currently being used in the management of ED. However, they are not completely effective in curing this condition. Platelet-rich plasma (PRP) contains several bioactive substances that help in cell proliferation, vascular formation, collagen production and tissue repair and might have a potential for treating ED.
Aim
To assess the efficacy of platelet-rich plasma (PRP) in the treatment of erectile dysfunction (ED).
Method
Study Design
- Systematic review and meta-analysis of randomized controlled trials (RCTs)
Treatment Strategy
- Relevant RCTs were identified using the databases of PubMed, Cochrane. Embase and Web of Science
- The data was analysed and differences in minimal clinically important difference (MCID) and International Index of Erectile Function (IIEF) between baseline and the 1st, 3rd and 6th month was evaluated
Endpoints
- Difference in MCID from baseline at 1st, 3rd and 6th month
- Difference in IIEF from baseline at 1st, 3rd and 6th month
Results
- Meta-analysis included 4 RCTs comprising 343 men with ED
- The therapeutic effect of PRP was superior and significant in the 1st and 6th month as compared to placebo (p=0.03 and p=0.008 respectively), as indicated by the difference in MCID
- The difference in MCID was insignificant at 3rd month; p=0.19
- Statistically significant differences in IIEF were noted at all the 3 timepoints in the PRP groups as compared to placebo; highlighting the superior efficacy of PRP after 1, 3 and 6 months of treatment (p<0.00001)
Conclusion
- Treatment with platelet-rich plasma (PRP) demonstrated significant improvements in men with erectile dysfunction as compared to placebo and may be a promising alternative in the treatment of ED.
