ARMYDA-4 RELOAD
Clopidogrel reloading provides no overall benefit in patients on chronic clopidogrel therapy & undergoing PCI ; however beneficial in ACS subgroup: ARMYDA-4 RELOAD
Aim
To evaluate the efficacy and safety of clopidogrel reloading in patients undergoing percutaneous coronary intervention (PCI) & on chronic clopidogrel therapy
Patient Profile
Patients receiving chronic clopidogrel therapy (>10 days) & undergoing PCI (N = 503)
Study Groups
- Reload group: 600 mg clopidogrel loading dose 4-8 h before PCI (n = 252)
- Placebo group (n = 251)
Primary Endpoint
Mortality, myocardial infarction (MI), or target vessel revascularization at 30 days
Results
In the overall population, a non-significant reduction of 25% was observed in the reload group vs. placebo
Incidence of primary endpoint did not differ significantly in stable angina patients; whereas significant reduction of 66% was observed in patients with ACS
Safety
Bleeding risk similar in both groups (6% in both)
Conclusions
- Clopidogrel reloading did not provide any overall benefit in patients on chronic clopidogrel therapy undergoing PCI
- Significant benefit observed in subgroup of patients with ACS
